چکیده

THORACIC paravertebral block (TPVB) is the technique of injecting local anesthetic adjacent to the thoracic vertebra close to where the spinal nerves emerge from the intervertebral foramina. This results in ipsilateral somatic and sympathetic nerve blockade in multiple contiguous thoracic dermatomes above and below the site of injection. It is effective in treating acute and chronic pain of unilateral origin from the chest and abdomen. Bilateral use of TPVB has also been described. Our understanding of the safety and efficacy of TPVB has improved significantly in the last two decades, prompting its use in children and neonates and for surgical anesthesia.

Update on the role of paravertebral blocks for thoracic surgery: are they worth it?

PURPOSE OF REVIEW To consider optimal analgesic strategies for thoracic surgical patients. RECENT FINDINGS Recent studies have consistently suggested analgesic equivalence between paravertebral and thoracic epidural analgesia. Complications appear to be significantly less common with paravertebral analgesia. SUMMARY There is good evidence that paravertebral block can provide acceptable pain...

Can thoracic paravertebral block replace thoracic epidural block in pediatric cardiac surgery? A randomized blinded study.

To compare the outcomes of thoracic epidural block with thoracic paravertebral block for thoracotomy in pediatric patients. A prospective double-blind study. 60 pediatric patients aged 1-24 months, ASA II, III scheduled for thoracotomy were randomly allocated into two groups. After induction of general anesthesia, thoracic epidural catheter was inserted in group E (epidural) patients and thorac...

Update on thoracic paravertebral blocks.

Thoracic paravertebral block was widely practised at the beginning of the 20th century. It has enjoyed a renaissance in the past decade. This form of afferent blockade is the technique of injecting local anaesthetic into the thoracic paravertebral space. Thoracic paravertebral analgesia is mostly indicated for unilateral surgical procedures of the thorax and abdomen. Compared to the other avail...

Thoracic paravertebral block after thoracotomy: comparison of three different approaches.

BACKGROUND Thoracic paravertebral block (TPVB) is a regional block technique increasingly used for the early management of post-thoracotomy pain. We compare three different postoperative analgesic approaches based on TPVB: anesthetist, anesthetist plus surgeon, and surgeon. MATERIALS AND METHODS We randomized 54 patients undergoing elective thoracotomy to three different postoperative analges...

Insertion of paravertebral block catheters intraoperatively to reduce incidence of block failure.

Paravertebral block catheters are a recognized method of regional pain control after a thoractomy. We describe a technique of insertion with the belief that it provides a superior positioning of the paravertebral (extra-pleural) catheter resulting in a better distribution of the local anaesthetic and better pain control.

[Clinical application of thoracic paravertebral anesthetic block in breast surgeries].

INTRODUCTION optimum treatment for postoperative pain has been of fundamental importance in surgical patient care. Among the analgesic techniques aimed at this group of patients, thoracic paravertebral block combined with general anesthesia stands out for the good results and favorable risk-benefit ratio. Many local anesthetics and other adjuvant drugs are being investigated for use in this tec...
